This Privacy Policy describes how we collect, use, disclose, and safeguard the personal information of our Clients and Personal Support Worker contractors (“PSWs”) in the course of operating our care coordination services in Ontario, Canada. “Personal Information” means any information about an identifiable individual. This does not include business contact information where collected for business purposes. The types of Personal Information we collect are described below.

a) From Clients and Their Representatives:

b) From Personal Support Workers (PSWs):

· Banking Information (for payment processing)
· Proof of Insurance

c) Automatically (via our website/platform):

- How We Use Personal Information
We use Personal Information only for the purposes for which it was collected, or with your consent.
a) For Both Clients and PSWs:
· To provide, operate, and improve our care coordination platform and services.
· To communicate with you regarding your account, assignments, schedules, service updates, and company information.
· To ensure the safety, security, and integrity of our services for all parties.
· To comply with our legal and regulatory obligations.
· For internal business purposes, such as data analysis and service enhancement, using aggregated and anonymized information where possible.
b) Specific Uses for Client Information:
We use Client Personal Information specifically to:
· Assess care needs and preferences to facilitate suitable matches with PSWs.
· Schedule and coordinate service visits.
· Process payments and invoices for services rendered.
· Maintain ongoing communication about service delivery.
c) Specific Uses for PSW Information:
We use PSW Personal Information specifically to:
· Verify identity, qualifications, certifications, and conduct screening (including criminal record and health checks).
· Assess suitability for assignments and match with Clients.
· Manage the Contractor relationship, including scheduling, assignment offers, and contract administration.
· Process payments for services provided.
· Maintain records required for insurance and regulatory compliance.
- How We Share and Disclose Personal Information
We do not sell your Personal Information. We only share it in the following limited circumstances:
· With Consent: We share a Client’s necessary information (e.g., name, address, care needs) with a PSW to facilitate a specific assignment. We share a PSW’s necessary information (e.g., name, qualifications, photo) with a Client for the same purpose.
· Service Providers: We use trusted third parties who assist us in operations (e.g., payment processors, cloud hosting, scheduling software). These providers are contractually obligated to protect your information and use it only for the services they provide to us.
· Legal Obligations: We may disclose information if required by law, such as in response to a subpoena, or to comply with a regulatory investigation.
· Business Transfers: In the event of a merger or sale of the Agency, information may be transferred as a business asset.
- Obtaining Consent
We obtain your express written or digital consent at the time of collection (e.g., through our service agreements and application forms). You may withdraw your consent at any time by contacting us, subject to legal or contractual restrictions and reasonable notice. Withdrawing consent may affect our ability to provide you with our services.
